If you answered ‘No’ to all of the questions, you can either print the questionnaire, sign it, and bring it with you at our first meeting, or I will ask you fill out a copy when we meet.
If you answered ‘Yes’ to any of the questions, that does NOT mean personal training is not for you. In fact, you probably stand to benefit from it even more! But before we begin, please print out the following release form and have your doctor sign it following a physical examination. It will ensure your safety and give us more information about the kinds of exercise that are best suited to you :Physician Letter
Motivational Readiness:
Do you have: At least 3 hours per week to dedicate to exercise?
Access to a form of cardiovascular exercise?
Are you prepared to: Make physical activity a priority, like eating and sleeping?
Commit to meeting with me at least once per week for a month?
Fill out a basic meal tracking form every day so that I can help guide you in your nutrition?
Respect yourself and the work you are doing by feeding your body what it needs to recover and have energy for your increased activity level?
Respect me by honoring appointments, except in extenuating circumstances, giving 36 hours notice when reasonably possible?
MOST IMPORTANTLY:
Do you know WHY you want to become more active?
(Hint: If your only answer is, “To look better,” think harder.)
